Stoma Care Nurse (Community and Hospital) - Midlands

BRIEF JOB SUMMARY:

Deliver high quality, evidence based care to all patients who require surgery, and who have or may require the formation of a stoma – both within the hospital and the community setting. You will also provide expert advice and education for all staff within the hospital and community. You are responsible for reviewing, auditing and improving the quality of care delivered.

MAIN DUTIES AND RESPONSIBILITIES

  • Assess, plan, implement and evaluate individualised nursing care of the patient with a stoma or fistula
  • Act as the patients’ advocate, through effective support and information enabling them to make informed choices and decisions concerning treatment
  • Act as a resource of information for cancer screening to the appropriate client group
  • Liaise with appropriate hospital staff and community teams to facilitate the provision of high quality care and a seamless service to all patients with a stoma.
  • Maintain accurate records in accordance with current best practice
  • Establish and maintain good working relationships with the multidisciplinary team and key opinion leaders
  • Act as an expert resource for nursing staff involved in the care of patients with stomas/fistulae/colorectal disease,
  • Be aware of current initiatives, research and development in the field of colorectal and stoma care nursing
  • Identify own educational needs in relation to the role and set objectives with manager(s) accordingly
  • Maintain the standards set by the NMC for continued effective registration,
  • Be an autonomous and accountable practitioner
  • Act within the boundaries of the Code of Conduct and Scope of Professional Practice, as defined by the Nursing & Midwifery Council (NMC)
  • Work in accordance with policies and guidelines
  • Participate in research initiatives applicable to the service
  • Provide clinical leadership and direction in colorectal/stoma care with the multidisciplinary team
  • Monitor standards and participate in audit as a means of quality assurance and continuous improvement
  • Participate in the service review process and data and reports as required
  • Maintain a flexible approach and develop the role in accordance with the requirements of the service
  • Participate in assessing, planning, implementing and evaluating the objectives of the colorectal and stoma services
  • Implement specialised educational programs for pre and post registration nurses and members of other disciplines
  • Be conversant with statutory training requirements and to attend update sessions as required
  • Maintain a safe clinical environment for patients within the bounds of responsibility
  • Be aware of the budgetary constraints, ensuring cost effective management of this budget
  • Have a sound understanding of the broad business and organisation objective
  • Carry out the responsibilities with due regard to equal opportunities policy
  • Act in accordance with the requirements of the Data Protection Act and patient confidentiality

Essentially you will maintain excellent clinical and professional standards of work, be self motivated to and able to work to set objectives. You will be able to manage your time and resources efficiently together with possessing excellent communication and interpersonal skills. You will have the determination to succeed and possess a good knowledge of primary care patient management systems.

This role requires flexibility in working patterns that involve travel within an identified region/territory and the postholder would be required to attend regional and company meetings which may necessitate occasional overnight stays. From time to time you may be required to cover other regions/ territories.

You will behave in a professional manner at all times and you will build professional relationships with patients and Health Care Professionals that will enhance the Company’s quality image.

ESSENTIAL CRITERIA:

  • Proven post registration experience
  • NMC Registered
  • Stoma or Colorectal background.
  • Commercial awareness
  • Excellent communication and interpersonal skills
  • Flexibility in order to meet patient and service needs
  • Ability to plan, implement and manage own time effectively
  • Dynamic and forward thinking
  • Determination to progress professionally in Stoma/Colorectal nursing
  • Ability to interrogate data and formulate advice, plans of action and reports
  • Confidence to present a positive image of self and the Company
  • Able to influence others
  • Flexible to meet patient and service needs
  • IT skills, including excel
  • Ability to work autonomously as well as part of a team
