Clinical Support Worker

To work as a member of the Stoma Care Team which provides assessment, planning and implementation of stoma care to patients and carers. To assist in the delivery of high standards of care to the individual under the direction of a qualified stoma care nurse, carrying out a range of duties in the hospital setting.

  • Assist under supervision in the assessment, planning, implementation, and evaluation of individualised nursing care of the patient with a stoma or fistula.
  • Develop a general understanding of stoma care core tasks and functions whilst identifying personal development needs.
  • Liaise with appropriate hospital staff to facilitate the provision of high quality care to all patients.
  • Observe and report any changes in the patient’s wellbeing to the SCN.
  • Observe and report any changes to patients stoma and surrounding skin to SCN
  • Replace and replenish stoma care stock to individual patients and the wards and undertake the ordering of stock for the stoma care department.
  • Assist in budget management.
  • Ensure effective communication with patients, relatives, carers and other professionals and agencies.
  • Ensure confidentiality and safe keeping of clients records.
  • Maintain accurate records in accordance with current best practice.
  • Work in accordance with policies and procedural guidelines.
  • Undertake the administrative support for the Stoma Care Nursing Team.
